In-Home services are provided by employees of The LIFE Center for Independent Living and consumers are assigned to In-Home Aides.

CDS - Services

CDS (Consumer Directed Services) is a program when the consumer hires you as an attendant for them and they direct their own care.  This can be a relative, friend, or family member as long as they are not a spouse.

Print, complete and bring to the Center, all of the following (except for the Training Manual and Payroll Calendar)
     - Attendant Application
     - Attendant Care Contract
     - Employment Eligibility Verification
     - Worker Registration
     - Arrest Notification

     - Federal W-4 Form
     - Missouri W-4 Form
     - Direct Deposit Enrollment
     - Focus Card Enrollment Form
     - Consumer/Attendant Training Manual
