Needing Medical (Non-Emergency) Transportation?*

Transportation routes are filled on a first come, first served basis so please call as far in advance of your appointment as possible in order to increase the likelihood that we are able to meet your transportation needs.

*Limited to elderly/disabled patients only. Patients utilizing other home care providers (not including L.I.F.E., Inc. or LIFE In-Home) are encouraged to contact SMTS @ 573-431-5765. Elderly/disabled patients WITHOUT a home care provider can call the L.I.F.E., Inc. office at 573-756-4314. NO same-day appointments will be allowed (unless someone else on today's route had a cancellation). Please call as far in advance as possible to schedule your ride. Priority will be given to doctor's visits. Our transportation program currently runs Monday through Friday as of March 2024.

Notifying the Public of Rights under Title VI

If you believe you have been discriminated against on the basis of race, color, or national origin by LIFE Inc., you may file a Title VI complaint by completing, signing, and submitting the agency’s Title VI Complaint Form.

How to file a Title VI complaint with LIFE Inc.:

  1. To obtain a Complaint Form, please contact Pauline Niehaus at LIFE Inc., 573-756-4314 ext. 115.


  1. In addition to the complaint process at LIFE Inc., complaints may be filed directly with the Federal Transit Administration, Office of Civil Rights, Region VII, 901 Locust Street, Suite 404, Kansas City, MO64106 or call1-800-368-1019.


  1. Complaints must be filed within 180 days following the date of the alleged discriminatory occurrence and should contain as much detailed information about the alleged discrimination as possible.


  1. The form must be signed, dated, and include your contact information.
