Consumer Directed Services
To qualify for this program you must be Medicaid eligible, have a physical disability, need assistance with daily tasks and be able to direct your own care. This program allows you to select and hire the attendant of your choice.
TAP Program
A telephone and Internet program is available to help you if you have difficulty using traditional phone or cannot see a computer screen to access the Internet
Circuit Breaker
We assist individuals who qualify for the circuit breaker tax/rent to complete their tax forms. You or your spouse must be disabled or over the age 65 with a combined annual income of less than $25,000 to qualify. You may be entitled to a refund of all or part of your real estate property tax or up to 20% of the yearly rent paid.
Ramp & Home Modifications
The Center will assist in building ramps and in completing home modifications for a safe and accessible environment in which to live. Contact Gary Copeland at the Center for more information.
Medical Equipment Loan
A variety of medical equipment is available to loan on a short-term basis.
Medication Assistance Programs
The Center will assist those who do not have prescription drug coverage to access pharmaceutical companies for free or reduced-cost medications. The individual's physician will have to be willing to assist in completing the application.
Community Work Incentive Coordinator, (CWIC)
The Community Work Incentive Coordinator (CWIC) works hand-in-hand to assist individuals with disabilities gain information on work issues and its impact on their benefits. The CWIC provides advice on SSDI work Incentives such as 9 month trial work period, extended period of eligibility, continuation of Medicare. Impairment-Related Work Expense, Subsidy, SSI Base Rate and calculation, earned income exclusion, 1619B extended Medicaid coverage and PASS plan.
Individual Education Plan (IEP)Assistance
Available for parents and children. Two MPACT certified Independent Living Specialist are available to help individuals understand the IEP process, the rules, and regulations.
Hear Now
This program assists individuals with hearing loss to obtain hearing aids for a small application fee. Individuals must meet an income guideline and have no insurance coverage for aids. A local hearing aid provider must agree to do the testing and fitting for free or at a reduced cost.
Medi-Vial of LIFE
LIFE, in partnership with Medicate Pharmacy, offers Medi-vials that hold pertinent medical information. There is a window cling for the front door and a magnet for the refrigerator which alerts medical personnel that pertinent medical information is in the refrigerator. The vial sheets ask questions about medicines, emergency contacts, listing of disabilities, etc. If you or someone you know would like to obtain one of these vials, contact the Center or your local USA Drug.
Transportation
The L.I.F.E. Center will has a full time van running between the hours of 8:00a.m. and 4:30 p.m. To schedule rides call the Center at 573-756-4314 or 1-800-596-7273
Internet Access
If you are interested in accessing the Internet and do not have a computer, the L.I.F.E. Center has a computer set up for your use. Please call 573-756-4314 to schedule ahead of time.
Hepatitis Support Group
A Hepatitis Support Group will be held the first Tuesday of every month at the L.I.F.E Center 1109 Ste.Genevieve Ave. from 6:00 pm to 7:30 pm. For more information call 573-756-4314 or 800-596-7273
